1. Eligibility

The CIS Community Card is available to eligible members of the CIS Career Community, including eligible current students/participants, alumni/previous participants, staff/faculty, and trainers/instructors, subject to verification by CIS/MUC.

2. Application and Verification

Applicants must provide accurate and complete information. CIS/MUC reserves the right to verify the information provided with the relevant department. Submitting an application does not guarantee approval or issuance of the CIS Community Card. Career Hub will verify eligibility before requesting payment.

3. Card Fee and Payment

The CIS Community Card fee is $20. Payment instructions will only be provided after eligibility has been confirmed. Card production will proceed after payment has been confirmed.

4. Card Issuance and Collection

Each approved applicant will receive a unique CIS Community Card. The preferred collection location selected in the application is subject to confirmation by Career Hub. Applicants will be notified when their card is ready for collection. The card must be collected according to the approved collection procedure.

5. Use of the Card

The card is personal to the approved cardholder and may not be transferred or shared with another person. The cardholder may be required to present the card when using a partner offer. Partner offers are subject to the individual partner’s terms, conditions, availability, and validity period.

6. Partner Offers and Discounts

Discounts and offers are provided by participating partners and may change from time to time. CIS reserves the right to add, remove, or modify participating partners and their offers. CIS is not responsible for changes made by individual partners to their offers, availability, or conditions.

7. Personal Information and Photo

By submitting the application, the applicant consents to CIS/MUC using the information and photo provided for eligibility verification, card production, card management, communication, and distribution, subject to applicable CIS/MUC privacy requirements.

8. Cancellation / Deactivation

CIS/MUC reserves the right to cancel or deactivate a card if the information provided is found to be inaccurate, the applicant is found to be ineligible, or the card is misused.

9. Updates to the Terms and Conditions

CIS/MUC may update these Terms and Conditions when necessary. Updated terms will be made available through the official CIS channels.
